Elección del implante intraarticular en el tratamiento de las lesiones del ligamento cruzado anterior
Los autores hacen una revisión de los diferentes implantes usados para
la sustitución intraarticular del ligamento cruzado anterior (LCA). Se analizan los injertos
autólogos, los materiales sintéticos y los aloinjertos tendinosos. Nos encontramos
ante un tema sujeto a controversia, aunque la tendencia actual es el uso de autoinjertos
de hueso -tendón rotuliano- hueso. No obstante, ninguno de los tejidos sustitutivos
puede remedar por completo la anatomía del LCA original.The authors review the different implants used for the intra-articular
substitution of the anterior cruciate ligament (ACL), analyzing the autologous grafts,
synthetic materials and tendinous allografts. We find ourselves facing a controversy,
although the present tendency is the use of bone-patellar tendon-bone autografts. None
of the substituted tissues, however, can completely resemble the anatomy of the original
ACL
Image quality loss and compensation for visually impaired observers
The measurement and modeling of image quality are aimed to assist the design and optimization of systems, typically built for ‘normal’ observer vision. But in reality image viewers rarely have perfect vision. There have been few attempts and no universal framework for measuring image quality loss due to visual impairments. The paper presents initial experiments designed to measure still image quality losses, as experienced by observers with visual accommodation problems, by proposing modifications to the Quality Ruler method described in ISO 20462-3:2012. A simple method is then presented, which compensates directly on the display for some of the quality lost due to the impairment. It uses a purpose-built image equalization software. The compensated image is finally examined in terms of quality gained. The losses and gains in image quality are measured on a Standard Quality Scale (SQS), where one unit corresponds to 1 JND. Initial results show that the quality lost due to visual accommodation impairments can be accurately measured with the modified ruler method. The loss is scene-dependent. Partial or full quality compensation can be achieved for such impairments, using image contrast equalization; the level of quality gained also scenedependent
Artropatía neuropática diabética múltiple: aportación de un caso
Se presenta un caso de artropatía neuropática múltipl e en un varón de
35 años de edad afecto de una diabetes mellitus tipo I grave y mal controlada, de larga
evolución. La artropatía debutó a los 30 años con una fractura espontáne a del cuello
de l astrágal o y fractur a de l maleól o media l tra s un traumatism o bana l de tobillo .
Posteriormente , cuatro años y medio más tarde, presenta una fractura espontánea del
calcáneo contralateral y tres mese s despué s una fractura del platillo tibial interno de
la rodilla izquierda. Los autores analizan los factores de riesgo para desarrollar una artropatía
neuropática severa, así como el tratamiento más apropiado.We present a cas e of multipl e neuropathi c arthropaty in a 35 year-old
male with a grave and poorly controlled mellitus diabetes Type I, of long duration. The
arthropaty began at age 30 with a spontaneous fracture of the neck of the talus and
fracture of the medial maleolo following a slight trauma of the ankle. Four and a half
year s later he presented a spontaneous fracture of the contralateral calcaneous, and
three months later a fracture of the internal tibial plateau of the left knee. The authors
analyze the risk factors for developing a severe neuropathi c arthropaty, as well as the
most appropriate treatment
Análisis histológico del crecimiento tisular en la fibra de carbono y en la prótesis de polietilenotereftalato cuando se utilizan como sustitutos del ligamento cruzado anterior
Se realiza un análisis histológico del crecimiento tisular en la fibra de
carbono y en la prótesis de polietilenotereftalato cuando se utilizan para reemplazar al
ligamento cruzado anterior (LCA). La fibra de carbono induce la formación de un tejido
conectivo organizado que se extiende siguiendo la dirección de las fibras, dando lugar a
un tejido similar histológicamente al ligamento en las zonas donde se ha conservado la
disposición longitudinal de los filamentos de carbono. En las zonas en las que los filamentos
se han roto, la inducción fibroblástica es importante pero desordenada, dando
lugar a un tejido conectivo irregular. La prótesis de polietilenotereftalato actúa como
inductor de una fuerte reacción inflamatoria de tipo granulomatoso, dando lugar posteriormente
a cicatrices fibrosas, con un tejido conectivo irregular. Tan solo a nivel de la
vaina periprotésica se aprecia una disposición en paralelo que recuerda al ligamento.
Los autores dudan del efecto funcional del crecimiento tisular en los ligamentos sintéticos
cuando se utilizan como sustitutos del LCAWe have carried out and histologic analysis of tissue ingrowth in carbon
fiber and polyethylene terephthalate ligament prostheses when used to reconstruct the
anterior cruciate ligament (ACL). Carbon fiber induces the formation of a connective
tissue so that it extends in the direction of the fibers, producing a tissue histologically
similar to the ligament in the zones where the longitudinal disposition of the carbon filaments
are conserved. In the zones where the filaments are broken, fibroblastic induction
is important but disordered, producing an irregular connective tissue. Polyethylene
terephthalat e ligament prostheses acts as an inductor of strong inflammatory reaction
(granulomatous type), in turn producing fibrous scars, with an irregular connective tissue.
Only at the periprosthetic level, a parallel disposition is observed, resembling the
ligament. We doubt about the functional effect of the tissue ingrowth in synthetic ligaments
when used as substitutes for ACL
Reparación de las insuficiencias crónicas del ligamento cruzado anterior mediante plastia autóloga con hueso-tendón patelar-hueso y malla de poliéster en doble fascículo : estudio preliminar
Se presenta una nueva técnica de reconstrucción de las lesiones crónicas
del ligamento cruzado anterior (LCA) en sus dos fascículos. Se estudian 30 paciente s
intervenidos con un promedio de seguimiento de 19 meses. Se utiliza hueso-tendón patelar-hueso
(H-T-H) reforzado con una malla de poliéste r en doble fascículo. El H-T-H,
reforzado con la malla de poliéster, pasa a través de una tunelización isométrica tibial
anteromedial y una tunelización isométrica femoral posterosuperior , mientras que el
resto de la malla de poliéste r pasa over-the-top al interior de la articulación para anclarse
en un punt o tibial posterolateral. Se han obtenido 26 casos de resultados excelente
s y buenos, 3 casos regulare s y 1 malo (por rotura de la plastia). Con esta técnica
se consiguen unas condicione s mecánicas, biológicas y quirúrgicas que permiten una
rehabilitación precoz y un funcionalismo rápido.A new technique for the reconstruction of chronic tears of the anterior
cruciat e ligament (ACL), that attempt to reconstruc the two bundles, is presented. 30
patients underwent surgery with a mean follow-up of 19 months. Bone-patellar tendonbone
is used, augmented with a polyester mesh. Bone-patellar tendon-bone augmented
with a polyester mesh is passed through an anteromedial tibial isometric tunne l and a
posterosuperior femoral isometri c tunnel, whil e the rest of the polyeste r mesh passe s
over-the-top to the interio r of the joint in orde r to attach at a posterolatera l tibial
point. We hav e obtained 26 case s of excellent or good results, 3 fair and 1 poor . We
think that with this technique we obtain certain mechanical, biological and surgical
conditions that allow early rehabilitation and rapid functioning
